- In order to delivery high quality healthcare services we need to record information relating to their health history, family history and medication in order to inform decision making. Personal demographic information is needed to support good communication with our patients, their GP and any other healthcare professionals involved in their care.

- Our registered patients may have queries during their care journey so reference needs to be made to healthcare information held on record. Similarly, if a complaint is received reference will also need to be made to the healthcare record. Where queries are received from a third party (e.g., insurance company) patient consent will be sought before these are responded to.

How long we keep information
Patients are marked as ‘inactive’ on our clinical software system on completion of their episode of care. In line with BMA guidance records will be retained for 8 years after the conclusion of treatment or death of the patient. Complete Cardiology holds electronic records only; once records reach the end of their retention period, they are made inaccessible to system users.

Duty of confidentiality
We are subject to a common law duty of confidentiality. However, there are circumstances where we will share relevant health and care information. These are where:
- you’ve provided us with your consent (we have taken it as implied to provide you with care, or you have given it explicitly for other uses);
- we have a legal requirement (including court orders) to collect, share or use the data;
- on a case-by-case basis, the public interest to collect, share and use the data overrides the public interest served by protecting the duty of confidentiality (for example sharing information with the police to support the detection or prevention of serious crime);
- If in England or Wales – the requirements of The Health Service (Control of Patient Information) Regulations 2002 are satisfied; or
- If in Scotland – we have the authority to share provided by the Chief Medical Officer for Scotland, the Chief Executive of NHS Scotland, the Public Benefit and Privacy Panel for Health and Social Care or other similar governance and scrutiny process.
